Depresyon
Günlük yaşantıda herkes kendisini zaman zaman mutsuz, karamsar ve üzgün hissedebilir
Her kendini mutsuz hisseden kişinin depresyona girdiğini söylemek doğru bir tanımlama olmayacaktır. Depresyonda olan birey, mutsuz ruh haline ek olarak yoğun bir çaresizlik, yetersizlik, değersizlik ve hayattan keyif alamama durumu yaşamaktadır. Bu durumu yoğun bir şekilde yaşayan ve depresyonda olan bireyler, çoğunlukla kurtuluş için tek çarenen intihar olduğunu düşünebilmektedir. Ayrıca bir kişiye depresyonda olduğunu söyleyebilmemiz için bu düşünce ve duyguları en az 2 hafta süreyle ve günün büyük çoğunluğunda yaşıyor olması gerekir.
Peki depresyon belirtileri nelerdir?
• Depresyonda olan birey, neredeyse her gün, günün büyük bir bölümünde çökkün duygudurum (umutsuz, karamsar, çaresiz, yetersiz, değersiz, sevilmez, kendini boşlukta hissetme durumu vs.) içindedir. Ergenlerde ve çocuklarda bu belirtiler kızgınlık ve öfke patlamaları şeklinde olabilmektedir.
• Bireyin işlevselliği büyük ölçüde bozulmuştur. Gündelik işlerini yapmakta zorlanır ve isteksizlik görülür. Okula/işe gitmek istemez ve yataktan kalkmak zorlaşır. Evde yapılması gereken işler varsa bu işleri yerine getirmede büyük ölçüde isteksizlik yaşar.
• Öz bakımda belirgin düzeyde bozulmalar meydana gelir. Duşa girmek, temiz kıyafetler giymek, dişini fırçalamak gibi basit eylemleri yerine getirmek zorlaşır.
• Neredeyse bütün aktivitelere karşı ilgi azalır. Önceden kitap okumayı seven, arkadaşlarıyla sık sık görüşmekten keyif alan ve sosyal aktivitelere katılmaktan hoşlanan birey, depresyona girdiği anda tüm bu etkinliklere karşı ilgisi azalır ve neredeyse tüm gününü evin içinde çoğunlukla da yatağında geçirir.
• İştahta azalma ya da artma görülür. Diyet ve egzersiz yapılmadığı halde kiloda belirgin bir düşüş söz konusudur. Özellikle çocuklar gelişim düzeyine göre düşük kiloya sahipse ve gerekli kilo alımı sağlanamıyorsa dikkat etmek gerekir.
• Uyku düzeninde bozulmalar görülür. Birey, aşırı uyur ya da tam tersi yoğun bir uykusuzluk durumu yaşar.
• Başkalarınca görünebilir düzeyde konuşmada ve hareketlerde yavaşlama (özellikle aile ve yakın arkadaş çevresi bu durumu fark eder).
• Enerjide düşüş, bitkinlik.
• Yoğun düzeyde suçluluk ve değersizlik duyguları.
• Düşüncede ve odaklanmada güçlükler.
• En önemli belirti ise tekrarlayıcı bir şekilde ölüm düşüncelerine sahip olmaktır. Tüm bunlara ek olarak bireyin ölüm isteği ve intihar planı varsa ve bireyin kan değerlerinden kaynaklanan bir problem söz konusu değilse, büyük bir ihtimalle depresyondadır diyebiliriz.
Unutulmamalıdır ki her birey zaman zaman bu belirti kümelerinden bir ya da birkaçını yaşıyor olabilir ancak depresyonda olduğunu söyleyebilmek için işlevsellik düzeyinin büyük oranda bozulmuş olması ve bu belirtileri en az 2 hafta süreyle yaşıyor olması gerekir. Her mutsuz olan ve gündelik işlerde ufak tefek aksamalar yaşayan bireyler için depresyona girmiştir diyemeyiz.
Depresyon Tedavisi
Özellikle ağır depresyon geçiren ve intihar düşüncesi olan bireylerin acil bir şekilde psikiyatr ile görüşmesi gerekmektedir. Depresyon tedavisinde ilaç çok önemli bir yere sahiptir. Bazen bireyler, ilaçların işe yaramadığını düşünerek erken bırakabiliyor ya da biraz iyileşmeye başlayınca ‘’artık iyileştim’’ diyerek doktor kontrolü olmadan ilaçları bırakabiliyor. Bu nedenle ilaçların mutlaka doktor kontrolünde alınması ve bırakılıyorsa da doktor kontrolünde bırakılması gerekmektedir. Depresyon zor bir rahatsızlıktır, bu nedenle ilaçlara ek olarak terapiye gitmek ve psikolojik destek alıyor olmak çok kıymetli bir yere sahiptir. İlaca ek olarak terapi desteği alıyor olmak depresyon tedavisinde etkinliği arttırmaktadır.
